چکیده

'Neurogenic thoracic outlet syndrome' is a poorly defined and controversial term for a group ofupper extremity nerve compression disorders.' The objective of this article is to summarize current knowledge, review controversies, and formulate a rational management plan for neurogenic thoracic outlet syndromes (NTOS). We will present a balanced perspective that considers thoracic surgical and neurological viewpoints. Thoracic outlet syndromes can be divided into four groups: arterial, venous, classic neurogenic, and non-specific neurogenic.2'3 The diagnosis of arterial and venous thoracic outlet syndromes is straightforward since characteristic clinical and radiographic findings are present. Classic NTOS is a diagnosis based on objective diagnostic features. Non-specific NTOS is a diagnosis based on suggestive symptomatology and exclusion of other entities. Vascular thoracic outlet syndromes are not discussed. Classic and non-specific NTOS will be reviewed separately.

Work-Related Neurogenic Thoracic Outlet Syndrome: Diagnosis and Treatment.

Outcomes of surgery for neurogenic thoracic outlet syndrome (NTOS) in workers' compensation are poor in a majority of patients, partly due to nonspecificity of diagnosis. Most cases have no objective evidence of the presence of brachial plexus dysfunction. Up to 20% of patients experience a new adverse event. Objective neurologic signs and electrodiagnostic evidence of brachial plexus dysfuncti...

Choosing Surgery for Neurogenic TOS: The Roles of Physical Exam, Physical Therapy, and Imaging

Neurogenic thoracic outlet syndrome (nTOS) is characterized by arm and hand pain, paresthesias, and sometimes weakness resulting from compression of the brachial plexus within the thoracic outlet. While it is the most common subtype of TOS, nTOS can be difficult to diagnose. Furthermore, patient selection for surgical treatment can be challenging as symptoms may be vague and ambiguous, and diag...

New Diagnostic and Treatment Modalities for Neurogenic Thoracic Outlet Syndrome

Neurogenic thoracic outlet syndrome is a widely recognized, yet controversial, syndrome. The lack of specific objective diagnostic modalities makes diagnosis difficult. This is compounded by a lack of agreed upon definitive criteria to confirm diagnosis. Recent efforts have been made to more clearly define a set of diagnostic criteria that will bring consistency to the diagnosis of neurogenic t...
